Frankly, Azure licensing is a hot mess and extremely expensive comparatively speaking, and OP has big time compliance to worry about when configuring it.ДЄctually I fully agree with you, however they seemed to indicate they where having an issue with users where having issues going from computer to computer when it comes to the Office suite that comes with O365. If they've currently got on-prem AD I might supplement it with Azure but I don't see a reason to replace it completely with the information he gave us. Honestly, I'm not sure what OP is trying to accomplish other than change for the sake of change. Much cheaper than rounding up separate solutions for all that stuff, plus you get the local office apps included in the subscription. That's what I do for us (Insurance) and it's only a couple bucks a month extra per user compared to the lower tier O365 licensing. I'd actually highly recommend they go the opposite route and look into Office 365 Enterprise E3 licensing, which includes features like email encryption and putting users on permanent litigation hold for archival purposes. It's a doctor's office, which means they need to be HIPAA compliant. This might actually save you money in the long run because licencing for users is less if you do not have a client applications. However since you are in a highly regulated industry you might just want your users to use the OWA for their Office needs rather that the client applications. Azure AD would probably be the easiest way to manage your environment and then you could lose your on premise DC.
